CLIA Intake Forms
If you would like to have an initial consultation with an expert staff member, please come into our office to fill out an intake form.
New clients are charged an initial admin fee is $60.00 payable by cash, check, or debit/credit card (there is a surcharge of 2.75% for electronic transactions). Admin fee can also be paid online: (please wait until you are scheduled an initial consultation before paying)

All forms are available to download for a legal consultation with one of our immigration lawyers or Department of Justice Accredited staff. If you have any questions email firstname.lastname@example.org for help.
With the COVID-19 pandemic completed forms can be:
- Emailed to email@example.com
- Mailed to our office 3047 N 70th St Lincoln, NE 68507, or
- Dropped off in our client drop off box located near the entrance of our office

Intake for those who have had an LPR (Green card) and are ready to apply for citizenship.
Fee waiver form attached (can decide to fill it out or waive it. If you do fill it out you must attach 2019 taxes and proof of benefits)
Intake if you have been a victim of domestic violence or other violence-related crimes. CLIA prioritizes these applications and upon submission, someone from our office will contact you.
